Physical Therapist

Provides services to students eligible for related services. Improves, develops,
restores or maintains a child’s sensory motor function in educational environments.
Functions as “related service personnel” under state and federal law and regulations.

Qualifications:
o Degree in physical therapy from an accredited program.
o Licensed to practice physical therapy in Utah (DOPL licensed)

Essential Job Functions:
oMust be able to perform the following job roles and functions:
o conduct appropriate evaluation of students referred for related services under
IDEA and prepare written reports of the evaluations and findings
o participate in meetings as member of the multidisciplinary team
o participate in development of IEP or 504 plans for eligible students
o provide direct and indirect physical therapy in educational settings/least
restrictive environments to children
o collaborate with other school personnel regarding physical therapy and the
students’ needs
o delegate activities to teachers, parents and paraprofessionals
o train/instruct teachers, parents and paraprofessionals in delegated techniques
and equipment use
o travel to and among schools to provide services
o maintain records of service provided
o lift, transfer and position children and equipment as necessary


Registered Behavior Technician (RBT)

Connected 2 Therapy is looking for Registered Behavior Technicians in a contractor capacity to provide support to students with the most severe behavior challenges. RBTs will work under the supervision of a board certified behavior analyst (BCBA). 

Essential Duties and Responsibilities:

  • Implement behavior strategies and provide one-to-one or small group instruction to students with challenging behaviors as designed by the BCBA and IEP team (e.g., physical aggression, self-harm, elopement, destruction of property, other dangerous behavior)

  • Deliver direct instruction to students in special education and general education settings 

  • Intervene directly with students in emergency situations (e.g., behavior crisis, injured student, fights, seizures, etc.) for the purpose of resolving immediate safety concerns

  • Assist with individualized assessments (e.g., curriculum-based, developmental, social skills), functional assessment

  • Collect data on behavior and-or academic target(s), develop data graphs, and regularly update progress monitoring

  • Monitor student attendance and disciplinary data (e.g., office referrals, suspensions, etc.)

  • Coordinate with BCBA and school staff to assist with the development of behavior interventions plans

  • Coordinate with BCBA to help determine the effectiveness of behavior plans

  • Train and model interventions to school-based para-educators to replicate duties after students are maintaining stable behavior.

  • Other duties as assigned



Contractor expectations:

  • Remain current in the field of behavior, including Safety Care training 

  • Demonstrate competence in learned technical skills

  • Respond appropriately to feedback and maintain or improve performance accordingly

  • Maintain professionalism and comply with applicable legal, regulatory and workplace reporting requirements (e.g., mandatory abuse and neglect reporting)

  • Maintain emotional boundaries (i.e., avoid dual relationships, conflicts of interest, social media contacts)

  • Must be able to work flexible hours based on student needs  

  • Must have reliable transportation to travel independently to various schools 


Licensed Practical Nurse (LPN)

Connected 2 Therapy is looking for a Licensed Practical Nurse (LPN) to join our team as an

Independent Contractor. Our team supports Charter Schools with all their Related Service

needs.

Qualifications

  • Licensed Practical Nurse (LPN) license

  • Have or able to obtain licensure with the Utah Board of Nursing

  • Have or able to obtain certificates and/or registrations as required by federal, state or local law(s)

  • Have or able to obtain a First Aid and CPR card

  • Pass a background check

  • Experience in a school setting working with special needs and/or medically fragile students preferred.


Essential Duties and Responsibilities

  • Performs direct nursing care as needed including but not limited to: medication administration, tube feedings, urinary catheterization, suctioning, oxygen administration, seizure and diabetic management under the supervision of the Registered Nurse.

  • Demonstrate knowledge of the Nurse Practice Act and practices within an LPN’s scope of practice.

  • Under the direction of a Registered Nurse, plan day-to-day routines, educational activities, student-specific nursing interventions, and special projects for the purpose of assisting in evaluating progress and/or implementing 504, behavior plan and IEP objectives.

  • Assists students with physical education and other recreational activities.

  • Assists teachers and specialists in the testing and assessment of student skills.

  • Records and maintains accurate test and developmental data on student performance.

  • Ensures student safety, self-care, health care, implements medical protocols, hygiene care, and mobility assistance services which may include lifting, feeding, toileting/diapering, and infection control.

  • Implements prescribed physical therapy/medical assistance and support duties as directed by a licensed healthcare professional.

  • Attends staff development classes to remain current on behavioral, medical and data collection procedures.

  • Maintains required and necessary nursing documentation and data collection.

  • Demonstrates skill in effective and appropriate verbal and written communications for a wide variety of recipients and in a wide variety of settings.

  • Demonstrates ability to effectively present information and respond effectively to questions, in one-on-one and small group settings, and to students, staff and parents. Consistent, daily and punctual attendance.

Registered Nurse

Connected 2 Therapy is looking for a Registered Nurse to join our team as an Independent

Essential Functions

  • Oversee student(s) who require complex health procedures, special therapy or

    specialized medical equipment/supplies to enhance or sustain their lives during the school day.

  • Administers medication and specialized medical treatment for the purpose of providing appropriate care for ill, medically fragile and/or injured children.

  • Collaborates with parents, students, health care providers, and/or other agencies for the purpose of promoting needed treatment, securing information and complying with legal requirements.

  • Develops health care plans for the purpose of addressing students' health needs and implementing health care directions from parents and/or physicians.

  • Participates in a variety of meetings, including IEP, (e.g. local/state health meetings, programs, 504, child studies) for the purpose of sharing information and/or improving skills/knowledge.

  • Adheres to all relevant laws relating to patient and student care, including nursing licensure laws, procedures of the Utah Nurse Practice Act, National Association of School Nursing Guidelines, and LEAs policies and procedures.

    Qualifications

  • Must be a Registered Nurse

  • Must possess a valid, unencumbered Utah Registered Nurse License

  • A minimum of one to three years’ experience working as a school nurse preferred.


Speech-Language Technician (SLT)

Connected 2 Therapy is looking for Speech-Language Technicians to work under the direction of a supervising speech-language pathologist and will need to follow the guidelines under the USBE SLT Handbook to obtain and maintain licensing.

Responsibilities

  • Support instructional and therapeutic opportunities in the education setting under the supervision of the SLP.

  • Deliver therapy according to the IEP, schedule and under the supervision of the SLP.

  • Assist with informal and formal documentation (i.e., logs) as directed by the SLP

  • Collect data and document student performance and report to the supervising SLP

  • Assist the SLP with scheduling, record keeping, material preparation

  • Represent yourself as working under the SLP

  • Assist with speech, language, and hearing screenings, without interpretation.

  • Create your own schedule under the guidance of the school special education director and SLP
